2023 RESPIRATOR FIT TESTING & MEDICAL CLEARANCE INFORMATION

Time slots are in 20-minute increments (2 people at a time)
Any individuals working with pesticides that require the use of respirators are required to be medically cleared and fit tested annually.
Medical clearance can be completed by a physician or licensed healthcare professional prior to the event. Individuals will need to complete the OSHA Respirator Medical Evaluation Questionnaire and review it with a physician or licensed healthcare professional.
The North Carolina Agromedicine Institute will be conducting the medical clearance and fit testing. Once you have signed up, the questionnaire will be mailed to you. Individuals are encouraged to complete the clearance form before the Fit Test Event to expedite the process.
Medical Clearance & Fit Testing will cost $45
Fit Testing will cost $25
Please bring a CHECK made payable to: ECU or have EXACT CASH amount.

Please bring the respirator you intend to wear when applying, mixing, and loading pesticides. PLEASE BE CLEAN-SHAVEN ANYWHERE THAT THE RESPIRATOR TOUCHES.
