Free Diabetes Education

Do you need help better managing your diabetes? Health Extension for Diabetes is for people who have been diagnosed with either type 1 or type 2 diabetes. The program provides participants with:

• diabetes education;

• self management resources;

• support to better manage diabetes and prevent complications.

Group members meet for 1 hour every other week for 8 sessions to learn about diabetes with optional support group meetings in between to share successes, and challenges.

During the program, group members also receive ongoing support and health education weekly. Complete Interest Form Here .
